These Terms of Use ("Terms") govern your access to and use of the PWP Health website at pwphealth.com (the "Site"). By using the Site, you agree to these Terms. If you do not agree, please do not use the Site.
Not medical advice, and not for emergencies
The Site provides general information about PWP Health and the services we offer. It does not provide medical or mental health advice, diagnosis, or treatment, and using the Site does not create a provider-patient relationship. Do not rely on the Site in place of care from a qualified professional.
The Site is not a crisis service. If you or someone you know is in crisis or may be in danger, call or text 988, the Suicide and Crisis Lifeline, or call 911. Do not use the Site's forms to report an emergency.
Who may use the Site
You may use the Site only if you can form a binding agreement with us, and only for lawful purposes. If you use the Site on behalf of someone else, such as when submitting a referral, you confirm that you are authorized to provide their information.
Information you submit
When you submit a message or referral, you agree that the information you provide is accurate to the best of your knowledge. Please do not include sensitive clinical details you would not want sent by email. Submitting a form does not guarantee that services are available or that a provider-patient relationship will be formed. How we handle what you submit is described in our Privacy Policy.
Acceptable use
When using the Site, you agree not to:
- Use it in any way that violates the law or these Terms.
- Interfere with or disrupt the Site or its security.
- Attempt to gain unauthorized access to any part of the Site or its systems.
- Use the Site to send unsolicited or harmful content.
- Copy, scrape, or misuse the Site's content except as allowed below.
Intellectual property
The Site and its content, including text, graphics, logos, and design, are owned by PWP Health or its licensors and are protected by intellectual property laws. You may view and print pages for your own personal, non-commercial use. You may not otherwise reproduce, distribute, or create derivative works from the Site without our written permission.

The Site may link to third-party websites and resources for your convenience, including the 988 Suicide and Crisis Lifeline. We do not control and are not responsible for those sites or their content, and a link does not mean we endorse them.
