PWP Health provides community-based services to consumers living with serious mental illness. We are currently in search of high performing Caseworkers to work at our Chicago IL location!
The ideal candidate must have excellent communication and documentation skills and possess an understanding of all mental health. The Caseworker will play an active part in the interdisciplinary team.
Caseworker responsibilities include:
Participating in the person-centered planning process. Providing direct home-based behavioral health services and supports, including therapy for the family.
Providing direct services for the family as indicated in the Person-Centered Plan.
Assisting the team leader in monitoring and evaluating the effectiveness of interventions as evidenced by symptom reduction and progress toward goals.
Assisting the team leader with consulting identified non-medical providers, engaging community and natural supports, and including their input in the person-centered planning process.
Meeting minimum billable units as assigned by the Program Director.
As a Caseworker, you will be hired at one of the below levels:
Qualified Mental Health Professional (QMHP)
Mental Health Professional (MHP)
Rehabilitative Services Associate (RSA)
Caseworkers can bill for the following services:
H2015- Community Support
T1016- Case Management: Mental Health
T1016 HS- Case Management: Client Centered Consultation
T1016 TS- Case Management: TLA ( excluding RSA)
H2011- Crisis Intervention ( excluding RSA)
**Please pay close attention to the qualifications (presented below) associated with the level to which you are applying (i.e., RSA, MHP, and QMHP). You will be asked to indicate your level during application submission, and you will be assessed at your respective level during the interview. **
Qualified Mental Health Professional or QMHP – You must meet one of the following:
A licensed social worker (LSW) possessing at least a master’s degree in social work and licensed under the Clinical Social Work and Social Work Practice Act [225 ILCS 20] with specialized training in mental health services or with at least 2 years experience in mental health services;
A licensed professional counselor possessing at least a master’s degree and licensed under the Professional Counselor and Clinical Professional Counselor Licensing Act
[225 ILCS 107] with specialized training in mental health services or with at least two years experience in mental health services;
A registered nurse (RN) licensed under the Nurse Practice Act [225 ILCS 65] with at least one year of clinical experience in a mental health setting or who possesses a master’s degree in psychiatric nursing;
An occupational therapist (OT) licensed under the Illinois Occupational Therapy Practice
Act [225 ILCS 75] with at least one year of clinical experience in a mental health setting; or An individual possessing at least a master’s degree in counseling and guidance, rehabilitation counseling, social work, vocational counseling, psychology, pastoral counseling, or family therapy or related field, who has successfully completed a practicum or internship that included a minimum of 1,000 hours of supervised direct service, or who has one year of clinical experience under the supervision of a QMHP.
- Any individual meeting the minimum credentials for an LPHA under this part is deemed to also meet the credentialing requirements of a QMHP.
Mental Health Professional or MHP – An individual who provides services under the supervision of a qualified mental health professional and who possesses: a bachelor’s degree; a practical nurse license under the Nurse Practice Act [225 ILCS 65]; a certificate of psychiatric rehabilitation from a DHS-approved program plus a high school diploma plus 2 years experience in providing mental health services; a recovery support specialist certified and in good standing with the Illinois Alcohol and Other Drug Abuse Professional Certification Association, Inc., plus one year experience in providing mental health services; an occupational therapy assistant licensed under the Illinois Occupational Therapy Practice Act [225 ILCS 75] with at least one year of experience in a mental health setting; or a minimum of 5 years supervised experience in mental health or human services. A supervised internship in a mental health setting counts toward the experience in providing mental health services. Any individual meeting the minimum credentials for an LPHA or QMHP under this Part is deemed to also meet the credentialing requirements of an MHP.
Rehabilitative Services Associate or RSA − An RSA must be at least 21 years of age, have demonstrated skills in the field of services to adults or children, have demonstrated the ability to work within the provider's structure and accept supervision, and have demonstrated the ability to work constructively with clients, treatment resources and the community. Any individual
meeting the minimum credentials for an MHP, QMHP, or LPHA under this Part is deemed to also meet the credentialing requirements of an RSA.
Training Requirements:
Must Complete All Agency initial orientation training and monthly training courses as required to include: HIPAA, Client Rights, Confidentiality, NCI, First Aid, CPR, and Blood Borne Pathogens training.
Service Definition Trainings: 3 hours service definition required components, 3 hours Crisis Response, 3 hours of I.T.P Instructional Elements
Training to be completed within thirty days of hire: Person Centered Thinking, Introduction to Motivational Interview, Cognitive Behavioral Therapy
