Care that meets people where they are.
PWP Health is a minority and women-owned mental health provider serving individuals, families, schools, and communities across Illinois. We build care around the person, not the paperwork.
Fewer handoffs, clearer next steps, and a steady hand through the hard places.
Mental health care is often hard to find and harder to navigate. We started PWP Health to change that. Our teams provide integrated, community-based care that treats the whole person, coordinates the moving parts, and stays with people as they move toward stability.
We work in clinics, in schools, and in the places people already live and work. Wherever someone starts, our job is to make the next step easier to find.
Who we serve
- Individuals seeking therapy, assessment, or ongoing support
- Families looking for care for a loved one
- Schools and youth-serving organizations
- Hospitals, clinics, and referral partners
Where we work
Illinois, in person and close to home. Care happens in our offices, in the community, and in the home when that is what a person needs.
How we work
Our approachWe listen first
Care starts by understanding a person's goals and history, not by fitting them into a program.
We make the path clear
Clear next steps from the first visit, so people always know what comes next and why.
We coordinate care
Fewer handoffs. We connect the therapy, the case management, and the community resources into one plan.
We stay with people
Stability takes time. We keep showing up, in the clinic and in the community, for as long as it takes.
